It is also her responsibility to make sure that the apartment is in a good living condition. Since your landlord was not able to give it to you then you have all the right to break your lease. All you need to do is to inform your landlord by giving a 30-days notice to vacate the unit sent via certified mail. If your landlord refuses to take you off your lease early then you can file a complaint against her.
