slumlord! wouldnt repair leaking ceiling for 2yrs

I moved into my apt 11/09 and moved out 11/11. During this time starting 1/10 my living room ceiling formed a water pocket everytime it rained. I contacted the landlord he said put a small hole in it and drain the water out. ok so I did. As months went by and the rainy days continued the leak gotten bigger. Called landlord again he sent his "maintanence guy" over who then tore off the paint from the wet area exposing black mold.  The leaking went on for a year with me just putting buckets under it when it rained.  I have asthma and my breathing has gotten worse.  So I felt I shouldnt have to pay my rent until he had it fixed. He evicted me and locked me out of my apt. He took my laptop computer, several floor cleaners, tool box for insurance I wouldnt break in.  The next day he let me get my stuff out after i had it all moved out we came to agreement and he let me move back in.  He had his maint guy come back and fix(TRY) the roof and cut a big hole in my living room (says to let it dry out).  The hole remaoned there for 6 more monthes before he tryn to evict me again cuz i wouldnt pay my full months rent.  I got a lawyer and a agreement was made id move out by 11/11.  Ive been living in a motel for 5 months now and cant get a place because he wont give me a referance plus the court records state i have 3 evictions, which he started and stopped 2 of them.  He kept my deposit of $2400 as well. I want to take him to court.
